Dan Rafael Azar M.D., M.P.H., Q.M.E. has been practicing Occupational Medicine in Silicon Valley since 1991 and has been the Medical Director of Alliance Occupational Medicine since its inception in 2001. Dr. Azar is a graduate of the Mount Sinai School of Medicine in New York City, and completed a residency in Occupational Medicine at the University of Pennsylvania, Philadelphia. Dr. Azar has a Master's Degree in Public Health, and is Board Certified in Preventive and Occupational Medicine. Dr. Azar is a California Qualified Medical Examiner and a Certified Medical Review Officer for drug testing. Dr. Azar is a member of the American College of Occupational & Environmental Medicine and was voted "Top Industrial Medicine Physician in Silicon Valley" by San Jose Magazine (February 2005). Dr. Azar is an accomplished speaker and has presented educational seminars to employers and employees on a wide range of health topics related to wellness, injury prevention, and disability management.
